School Forms for All Students

Forms for the 2019-20 school year are available in the Parent Portal. If you have not received your Community Login information by May 8, please contact the webmaster at webmaster@charchrist.com. There is one "Back-to-School" form per division. The form is required and due by Aug. 21, 2019. Please note, based on your selections you will be required to upload a doctor-signed document to complete the online form. For example, if you wish to give the nurses authorization to give your child over the counter medication, a Medication Authorization sheet is a required upload. Extended Day for Grades JK-5

Classroom space, computers and outdoor play facilities are available to children for homework, play and various other extracurricular activities. Special field trips on teacher work days and early dismissal days also contribute to making the Extended Day program a valuable opportunity for our students and their families.
