2024-25 Health Documents Upload Form

If your child has specific medical conditions or will be taking medication during the school day or if you would like the nurses to administer medicine during the school day, you will need to submit updated health forms signed by a doctor in order to complete your child's back-to-school form. If you have questions about health forms, please email nurse@charchrist.com.

Be sure you have all your necessary health forms ready to upload.

• Medication Authorization (for over-the-counter medication)
• Immunization records are required for all new students and all students entering grade 12, grade 7 and kindergarten. Please upload ALL pages of the immunization record. The immunization record must include the student's full name and date of birth. Please visit NCDHHS.gov for specific vaccine requirements.
• Allergy Action Plan
• Asthma Action Plan
• Diabetes Action Plan
• Seizure Action Plan
• Athletic Physical Form (for MS and US students participating in a CCS sport)

Complete the 2024-25 Athletic Authorization online form to upload your child's completed Athletic Physical Form. If you have any questions about athletic forms, please contact the Athletics Coordinator Kristy Burton at kristy.burton@charchrist.com.

You may upload all of your child's required forms as one scanned file or you may take a picture of each page individually and upload separate files.

Please DO NOT upload blank documents. Please note that schedules and class placement information will not be released until all completed health documents have been submitted.
