Back-to-School 2021

Back-To-School Form
The 2021-22 Back-to-School Form is required for all students, each year. If you have multiple children you can enter them all in one form. This form is due by July 28, 2021.

Be sure you have all your necessary health forms ready to upload.

• Medication Authorization (for over-the-counter medication)
• Immunization Record (for all new students as well as KG, 7th grade & 12th grade)
• Athletic Physical (for MS and US students participating in a CCS sport)
• Allergy Action Plan
• Asthma Action Plan
• Diabetes Action Plan
• Seizure Action Plan
